Health Insurance Requirements:
All enrolled students must have health insurance through an ACA accredited/approved health insurance plan while at É«ÇéÊÓƵ. Students may opt to enroll in the É«ÇéÊÓƵ Student Health Insurance Plan (OSHIP), or to waive OSHIP and maintain their own health insurance, as long as the coverage meets minimum requirements. For on-campus study, students are required to have health insurance that covers primary and specialty care services in Los Angeles County - having only urgent and emergency care coverage does not qualify for a health insurance waiver.
Opting to waive OSHIP must be done before the August 20, 2024 deadline in the fall to avoid being enrolled in and charged for the É«ÇéÊÓƵ Student Health Insurance Plan, managed by Aetna Student Health. Students without health insurance and who do not waive will automatically be enrolled in the Aetna Student Health Insurance Plan. There is an additional opportunity to waive or opt into OSHIP for the spring semester in the event that there are changes to a student’s insurance coverage. Waivers must be completed at . Health Forms:
All entering students are required to have completed the following forms prior to registration for classes: 1) Immunization Record (completed by a medical provider), 2) Health History, 3)Tuberculosis (TB) Screening Questionnaire, 4) Financial Responsibility for Treatment Agreement, 5) Health Physical (recommended for all students, required for NCAA and Club Sports athletes), 6) Authorization to Share information with Athletics (for NCAA and Club Sports Athletes only). Please read the instructions carefully as some forms need to be printed and signed, some need to be completed online, and some require a physician signature. International students who do not have access to may get copies sent to them electronically from Emmons Wellness Center and/or the International Programs Office. Completion of all health forms prior to matriculation are strictly enforced to ensure the health and well-being of all É«ÇéÊÓƵ students. Failure to comply with this policy will result in late fees and a hold on the student's account, suspending their meal plan and class enrollment.
Emmons Wellness Center uses the through their Electronic Medical Records system, Point&Click. This allows for all forms to be directly linked to the student's medical record. This system offers the highest level of health information protection and privacy. Students may then communicate with providers through this portal, as well as receive appointment reminders and visit follow-up information.
